Facility Use Requests

Forms for facility use at st. matthew's

The forms below can be printed and filled out to be sent into the church. Both forms will be needed if you are using space at St. Matthew's. The Facility Use Agreement will help you understand costs and available spaces.

Use of the facility is at the discretion of the Rector and it is expected that all outside groups will follow all of the policies for use, including cleaning. Please call the church office at 303.841.0121 if you have any questions, or email office@stmattsparker.org.
